Forms 2008-2009

Band and guard registration is almost here! You will need to print these forms, complete them and bring them to band and guard registration on Saturday, August 9. Registration is from 9 am until noon. The first form, the Activity Release Form is not required at registration, but it just may be the most frequently used form provided...note its location!

These forms are available in Microsoft Word and Adobe Acrobat formats. The Adobe Acrobat format will require you to have Adobe Acrobat Reader on your computer to read and/or print these forms.   Adobe Acrobat Reader is a free download from the Adobe website, provided below.
